Pediatric Home Service is launching our new Hospital to Home program, and we’re seeking a dedicated Medical Social Worker to help medically complex children safely transition from the hospital back to the place they belong—home.

THE POSITION

Why You’ll Love This Role:
As part of our Hospital to Home team, you’ll provide vital social work support to ensure timely hospital discharge, prevent rehospitalization, reduce emergency department visits, and promote school attendance. You’ll work closely with our clients, families, and our extended care team to ensure children thrive in their home environment.

SALARY RANGE

At PHS, we are committed to providing specialized care for medically complex children. Every role, from direct patient care to customer support, plays a vital part in improving the lives of kids and families. The impact you make is invaluable. We are dedicated to offering fair and transparent compensation to our team members. The range for this role is $27.16 to 32.60, based on experience and qualifications.
